  1. Introduction and scope
    Triple Health, PLLC and The Renewal Collective, LLC (d/b/a Triple Health Skin & Wellness) (collectively, “Triple Health,” “we,” “us,” or “our”) respect your privacy. This Privacy Policy explains how we collect, use, disclose, and protect personal information obtained through triplehealthclinic.com and the webpages, forms, communications, and online offerings operated under Triple Health Clinic, the Midlife Women’s Institute, Triple Health Club, and Triple Health Skin & Wellness (collectively, the “Website”). This Policy applies to personal information collected through the Website. It does not govern patient information maintained by Triple Health Clinic as part of a clinical record. Clinical records and patient-care communications are handled under applicable Iowa law, professional obligations, clinic policies, and patient consents. Independent professionals, organizations, and businesses listed or linked as resources are governed by their own privacy practices.
  2. Information we collect
    Depending on how you use the Website, we may collect: Contact and inquiry information, such as your name, email address, telephone number, and the contents of a message.
    Registration and preference information submitted when you request a guide, join a mailing list or waitlist, register for an educational offering or event, inquire about Triple Health Club, or otherwise communicate with us.
    Transaction, membership, and event information associated with purchases, registrations, recurring memberships, or participation. Payment-card information is generally collected and processed by a third-party payment provider rather than stored by Triple Health.
    Technical and usage information, such as Internet Protocol address, browser and device type, pages viewed, referring pages, approximate location derived from an IP address, dates and times of visits, and interactions with the Website.
    Cookie and similar-technology information used to operate and secure the Website, remember preferences, measure performance, or support embedded content.
    Information received from service providers or platforms you choose to use in connection with our offerings, such as scheduling, payment, membership, email, or event platforms.
  3. Website forms and health information
    General Website forms and ordinary email are intended for general inquiries and are not appropriate for emergencies, urgent concerns, detailed medical histories, prescription requests, or other sensitive health information. Please do not submit sensitive medical information through a general Website form or ordinary email.
    Current patients should use the patient portal or Triple Health’s designated secure communication system for patient-care communications. Submitting a Website form, downloading a guide, joining a mailing list, or contacting Triple Health does not establish a patient-provider relationship.
  4. How we use information
    Respond to questions, requests, appointment inquiries, and customer-service needs.
    Deliver requested guides, resources, newsletters, announcements, and educational materials.
    Administer Institute programs, Club membership, waitlists, events, registrations, purchases, and related communications.
    Maintain records of communications, transactions, and preferences.
    Operate, secure, troubleshoot, evaluate, and improve the Website and our offerings.
    Comply with legal, regulatory, professional, contractual, accounting, and record-retention obligations.
    Prevent fraud, misuse, security incidents, and other harmful activity.
  5. Email communications
    If you request a guide, subscribe to updates, join a waitlist, register for an offering, or otherwise opt in, we may send educational, administrative, or promotional email. You may unsubscribe from marketing emails using the unsubscribe link in the message or by contacting hello@triplehealthclinic.com. We may still send nonpromotional communications necessary to administer a transaction, registration, membership, or existing relationship.
  6. Text messages
    If you affirmatively consent to receive text messages, Triple Health may send messages consistent with the consent you provided. Message frequency may vary. Message and data rates may apply. Consent to marketing texts is not a condition of purchasing any product or service. You may opt out at any time by replying STOP or by making another reasonable opt-out request.     We do not sell mobile telephone numbers or text-message opt-in consent, and we do not share them with third parties for those third parties’ own marketing. We may provide information to vendors that help us deliver messages, only as reasonably necessary to provide those services or as required by law. Wireless carriers are not responsible for delayed or undelivered messages.
  7. Cookies, analytics, and similar technologies
    The Website and its hosting, security, form, scheduling, payment, membership, email, map, video, and social-media features may use cookies, pixels, local storage, or similar technologies. These technologies may collect technical and usage information to operate and secure the Website, remember preferences, measure traffic or performance, prevent abuse, or enable embedded content. Some technologies may be controlled by third-party providers and governed by their privacy policies.
    You can adjust cookie settings through your browser and, if available, the Website’s cookie controls. Blocking some technologies may affect Website functionality. Triple Health does not sell personal information for money. If Triple Health begins using personal information for targeted advertising or another activity that requires additional notice or choice under applicable law, this Policy and the Website’s controls will be updated as appropriate. The Website does not currently respond to “Do Not Track” browser signals.

    We may disclose the categories of information described above:
    To vendors and service providers that support hosting, forms, email, text messaging, information technology, security, analytics, document delivery, scheduling, payment processing, memberships, events, and related business operations.
    To professional advisers, auditors, insurers, or other parties that help us comply with legal, regulatory, professional, accounting, and risk-management obligations.
    When you direct us to do so or consent to the disclosure.
    To comply with law, legal process, licensing requirements, or a lawful government request.
    To protect the rights, safety, security, and property of Triple Health, our users, or others.
    In connection with a merger, reorganization, financing, sale, or transfer of all or part of a business, subject to applicable law.
    We do not sell personal information for money or disclose personal information to third parties for their independent marketing except with your consent. Service providers may process information on our behalf under their contracts and applicable privacy or security obligations.
  9. Third-party services and links
    The Website may link to or integrate with services operated by other organizations, including patient-portal, communication, supplement, financing, payment, scheduling, educational, event, social-media, map, video, and other services. When you leave the Website or use a third-party service, that organization’s privacy policy and terms govern its collection and use of information. Triple Health is not responsible for the privacy, security, availability, content, or practices of external services. A listing or link does not necessarily constitute endorsement of every product, service, statement, recommendation, or practice of the external organization or professional.
  10. Data retention and security
    We retain information for as long as reasonably necessary for the purposes described in this Policy, to maintain business and transaction records, resolve disputes, and satisfy legal, regulatory, professional, and contractual obligations. Retention periods vary according to the nature of the information and why it was collected.
    We use reasonable administrative, technical, and physical safeguards appropriate to the information we maintain. No website, email system, transmission method, or storage system can be guaranteed completely secure. Use the patient portal or designated secure communication system for sensitive patient-care communications.
  11. Children’s privacy
    The Website and Triple Health’s offerings are intended for adults and are not directed to children under 13. We do not knowingly collect personal information online from children under 13. If you believe a child has submitted personal information through the Website, contact us so we can evaluate and address the request.
  12. Your choices and requests
    You may ask to review, correct, or delete personal information you submitted through the Website, or withdraw a marketing preference, by contacting hello@triplehealthclinic.com. We will evaluate requests under applicable law and may need to verify your identity. Certain information may be retained when required or permitted for legal, regulatory, professional, security, transactional, or recordkeeping purposes.
    These Website privacy requests are separate from rights that may apply to patient records. Current or former patients should follow the clinic’s process for medical-record or patient-privacy requests. If an applicable state privacy law gives you additional rights, Triple Health will process a qualifying request as required by that law.
  13. Changes to this Privacy Policy
    We may update this Privacy Policy periodically. The revised version will be posted with a new effective date. Material changes may also be communicated through another reasonable method when appropriate.

  1. Acceptance of these terms
    These Website Terms of Use (“Terms”) apply to your access to and use of triplehealthclinic.com and Website content operated by Triple Health, PLLC and The Renewal Collective, LLC (d/b/a Triple Health Skin & Wellness). By accessing or using the Website, you agree to these Terms. If you do not agree, do not use the Website.
  2. Educational information only
    Website content is provided for general informational and educational purposes. It is not individualized medical advice and is not a substitute for evaluation, diagnosis, treatment, or advice from a qualified healthcare professional who knows your health history and circumstances.
    Health information changes over time, and Website content may not reflect the most recent research, guidance, product information, or regulatory developments. Triple Health does not guarantee that every item of content is complete, current, or appropriate for every person.
  3. No patient-provider relationship
    Viewing the Website, downloading a resource, joining a mailing list or waitlist, participating in general Club or Institute activities, submitting a general inquiry, or communicating through an ordinary Website form does not establish a patient-provider relationship with Triple Health or any clinician. A patient-provider relationship is established only after the clinic accepts you as a patient and the required clinical intake and consent process is completed.
  4. Emergencies and urgent concerns
    Triple Health Clinic is not an emergency medical service, and the Website is not monitored for urgent needs. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department. Do not use Website forms, ordinary email, social media, Club communications, or educational platforms for urgent medical concerns.
  5. Boundaries among Triple Health services
    Triple Health Clinic provides clinical services only within an established patient-provider relationship and within applicable professional licensure and scope. The Midlife Women’s Institute provides education. Triple Health Club is an education, community, and wellness membership and is not medical care. Institute or Club participation does not include diagnosis, individualized treatment, prescription management, laboratory interpretation, emergency support, or secure access to a medical provider unless a separate clinical relationship and appointment exist.
    Triple Health Skin & Wellness is operated by The Renewal Collective, LLC and provides skincare, aesthetic, and wellness services according to the training, licensure, and professional scope of the individual providing each service. Website information does not establish an esthetician-client or medical treatment relationship and does not replace service-specific screening, consent, or aftercare information.
    Triple Health Clinic is not a primary-care practice. Patients should maintain an appropriate relationship with a primary-care provider and other healthcare professionals for preventive care, acute illness, emergencies, and needs outside Triple Health Clinic’s scope.
    Clinical and telehealth services are available only where the treating clinician is authorized to practice and where Triple Health accepts the person as a patient. Website availability does not mean that any service is available in every location.
  6. No guarantees and individual results
    Health outcomes vary. Testimonials, examples, educational discussions, descriptions of services, and references to possible benefits do not guarantee a particular result. No statement on the Website should be interpreted as a promise that a condition will be diagnosed, improved, prevented, cured, or successfully treated.
  7. Products, supplements, and external resources
    References to supplements, skincare products, books, professionals, organizations, services, or other resources are provided for education or convenience. A reference, affiliate relationship, or link does not mean that Triple Health endorses every claim, opinion, product, service, treatment, or recommendation offered by a third party. Triple Health may receive compensation or other value from certain product or service relationships when disclosed. Supplements and wellness products are not appropriate for everyone and may interact with health conditions, medications, or other products. Consult an appropriate healthcare professional before making changes that may affect your health.
  8. Third-party websites and services
    Third-party websites, independent professionals, community resources, and external services are governed by their own terms, privacy policies, security practices, availability, and eligibility requirements. Triple Health is not responsible for their transactions, decisions, denials, financing, communications, products, services, recommendations, data practices, or content.
  9. Purchases, memberships, courses, and events
    Additional terms may apply to purchases, educational programs, Club membership, events, recurring payments, cancellations, refunds, and other offerings. Those terms will be presented at registration, enrollment, or purchase and are incorporated into these Terms by reference. If offering-specific terms conflict with these general Website Terms, the offering-specific terms control for that transaction or offering.
  10. Intellectual property and permitted use
    Unless otherwise stated, the Website and its original text, graphics, branding, downloads, handouts, guides, videos, and educational materials are owned by or licensed to Triple Health and are protected by intellectual-property laws. Triple Health grants you a limited, personal, nonexclusive, nontransferable, revocable license to access and use Website materials for your own noncommercial purposes.
    You may not reproduce, republish, sell, license, distribute, modify, remove ownership notices from, create derivative works from, or commercially exploit Website materials without prior written permission, except as permitted by law. Materials specifically labeled as printable or downloadable may be used personally in accordance with accompanying instructions.
  11. User submissions and acceptable use
    If the Website permits you to submit content, you represent that you have the right to submit it and that doing so does not violate law or another person’s rights. Do not submit confidential information about another person without authorization.
    You may not use the Website to violate law; infringe another person’s rights; submit false, harmful, abusive, or unlawful material; attempt unauthorized access; interfere with security or operation; introduce malicious code; scrape or collect information through unauthorized automated means; impersonate another person; or use Website content to provide medical care or commercial services without permission.

    To the fullest extent permitted by law, Triple Health and its owners, clinicians, employees, contractors, and agents will not be liable for indirect, incidental, special, consequential, exemplary, or punitive damages arising from or related to Website use, inability to use the Website, reliance on Website content, or third-party services. These limitations do not apply where prohibited by law and do not limit liability that cannot legally be limited.
  14. Indemnification
    To the extent permitted by law, you agree to be responsible for losses, claims, liabilities, and reasonable costs arising from your unlawful misuse of the Website, violation of these Terms, or infringement of another person’s rights. This provision does not require a consumer to indemnify Triple Health for Triple Health’s own unlawful conduct.
  15. Governing law and venue
    These Terms are governed by the laws of the State of Iowa, without regard to conflict-of-law principles. Any legal proceeding arising from these Terms or Website use must be brought in a court of competent jurisdiction serving Des Moines County, Iowa, unless applicable law requires otherwise.
  16. Changes, severability, and entire agreement
    We may update these Terms by posting a revised version with a new effective date. If any provision is found unenforceable, the remaining provisions will remain in effect, and the affected provision will be enforced to the maximum extent permitted by law. These Terms, the Privacy Policy, and any applicable offering-specific terms constitute the agreement governing Website use.
